The following forms are required for participation in the Take Off and Grow program. Please download and print them, then send them back to Take Off and Grow.

• Parent/Guardian Permission (.pdf)
• Secondary School Report (.pdf)
• Medical Examination (FAA Medical form 8508-8 and Medical Examination results - this form is only available at FAA Medical Examiner offices; if you are accepted into TOAG, you will be advised by TOAG staff if you are required to have a medical exam and where to go to get one)
